Vitamin D Deficiency Symptoms: What the Signs Actually Tell You

What are the symptoms of vitamin D deficiency?

Most people with low vitamin D have no symptoms at all. When symptoms do occur they are non-specific: persistent fatigue, aching in the lower back, hips or ribs, muscle weakness that makes stairs harder, muscle cramps, low mood and more frequent infections. Because these overlap with many other conditions, a blood test is the only way to confirm deficiency.

That last point is the one worth holding on to. Vitamin D deficiency is a biochemical diagnosis, not a clinical one. You cannot read it off how you feel.

Why vitamin D deficiency is usually silent

Vitamin D is a fat-soluble hormone precursor. The body makes it in the skin from UVB exposure, and it is converted first in the liver to 25-hydroxyvitamin D, or 25(OH)D, and then in the kidney to its active form. 25(OH)D is the circulating form with a half-life long enough to be a stable marker, which is why it is the one laboratories measure.

The body defends its calcium levels aggressively. When vitamin D falls, parathyroid hormone rises to pull calcium from bone and maintain serum calcium within range. That compensation works for a long time, which is exactly why deficiency can run for years without producing a single noticeable symptom. By the time musculoskeletal symptoms appear, the deficiency is usually established rather than early.

Yale Medicine states plainly that most people with vitamin D deficiency do not notice any symptoms. Cleveland Clinic describes the adult presentation as fatigue, bone pain, muscle weakness, muscle aches and mood change.

Every one of those is shared with iron deficiency, thyroid disease, sleep disruption and depression. This is the practical reason vitamin D is measured alongside other markers rather than on its own. Fatigue with a low 25(OH)D and a low ferritin is a different problem from fatigue with a low 25(OH)D alone, and it is worth understanding why ferritin can look normal when iron stores are not before attributing tiredness to any single cause.

What are the signs clinicians actually look for?

Clinicians look for a pattern rather than a single symptom: proximal muscle weakness such as difficulty rising from a chair, bone tenderness over the tibia or sternum, and diffuse musculoskeletal pain. In children, severe deficiency causes rickets with bowing of the legs and delayed growth. In adults, severe deficiency causes osteomalacia, a softening of bone.

The distinction between mild insufficiency and severe deficiency matters. Mild insufficiency is common and often asymptomatic. Osteomalacia and rickets are the endpoints of prolonged, severe deficiency, and they are uncommon. Medscape describes chronic muscle aches and pains as the adult presentation, with skeletal deformity confined to severe cases.

What is a normal vitamin D level?

Laboratories report 25(OH)D in nanograms per millilitre or nanomoles per litre. Widely used thresholds classify deficiency below 20 ng/mL, insufficiency between 20 and 29 ng/mL, and sufficiency at 30 ng/mL and above. Reference intervals vary between laboratories and between clinical guidelines, so results should always be interpreted against the range printed on your own report.

There is genuine disagreement in the literature about where sufficiency begins. Some bodies set the threshold for bone health at 20 ng/mL, others at 30 ng/mL. This is not an academic quibble. It is the difference between being told you are fine and being told you are insufficient on the same number.

Why living in the UAE does not protect you

The intuition that a sunny climate rules out deficiency is a poor guide. Cutaneous vitamin D synthesis requires UVB reaching bare skin, and several features of daily life in the Gulf work against that: extensive indoor and air-conditioned time during the hottest months, sun avoidance behaviours, clothing that covers most of the skin, routine sunscreen use, and higher skin melanin content in much of the resident population, which lengthens the UVB exposure needed for the same synthesis.

Vitamin D insufficiency is widely reported across Gulf populations in the published literature, and Cleveland Clinic Abu Dhabi maintains regional patient guidance on it. We have deliberately not quoted a single prevalence percentage here, because reported figures vary substantially by cohort, season, assay and threshold used.

Who should consider a vitamin D test?

Testing is most useful when a result would change what you do. That applies to people with unexplained persistent fatigue or diffuse musculoskeletal pain, people with limited sun exposure or extensive skin coverage, people with malabsorption conditions such as coeliac disease or inflammatory bowel disease, people who have had bariatric surgery, people already taking a supplement who want to confirm it is working, and older adults at risk of fracture.

Routine population-wide screening of people with no symptoms and no risk factors is not recommended by most guidelines. That is a meaningful constraint, and it is worth stating rather than glossing over.

What to do about a low result

A confirmed low 25(OH)D is treated by replacement. Dose and duration depend on how low the starting level is, body weight, absorption, and whether the cause is reduced synthesis or reduced absorption, so this is a decision for the clinician interpreting your result rather than a fixed number that applies to everyone.

Two practical points come up repeatedly. Vitamin D is fat-soluble, so absorption improves when it is taken with a meal containing fat, which is the substance behind the common question about the best time to take vitamin D. Timing within the day matters far less than consistency and taking it with food. Second, retesting after roughly three months of replacement is what confirms the dose is working, because response between individuals is variable. More vitamin D is not automatically better, and the distinction that matters is monitoring rather than dose alone. Replacement aimed at a defined target range, with a repeat 25(OH)D to confirm where you have landed, is a different proposition from prolonged high-dose supplementation taken without measurement. The latter can cause hypercalcaemia, and Cleveland Clinic lists nausea, increased thirst and urination, poor appetite, constipation, weakness and confusion among the symptoms of vitamin D toxicity.
